The Accountable Health Communities Health-Related Social Needs Screen Tool (AHC-HRSN) is a 10-question tool to screen for the quality and safety of a person’s living environment. It was developed by the Centers for Medicare and Medicaid Services (CMS) with the help of a panel of experts from around the country.
Who is this quiz for?
Millions of people struggle every day with the reality or uncertainty of being able to get the resources (e.g., healthy food, utility bills paid, adequate housing, etc.) absolutely necessary to live well. People of all different ages, backgrounds and life situations may at one time or another in their life face one of these issues.
